On February 19, 2018 a memorandum was forwarded to each of you regarding my annual performance appraisal. At that time, I inquired as to any changes that you would like to see in the performance evaluation form. The majority of you conveyed to me your satisfaction with the current tool. Therefore attached to this communication is a copy of the form that has been used in previous years.
